How your personal data is used
We collect your personal data to carry out the tasks described on this form. We store and process your data starting from when you send the form, for as long as it is needed for carrying out the tasks and also longer.
Most of your personal data that we have in our possession has been obtained directly from you.
The following personal data is collected with this form:

We protect your personal data by using several effective information security measures. The data is collected by means of an online form. Data is delivered from the form into our system using an encrypted HTTPS connection.
We always explicitly request for your consent when we collect special (sensitive) personal data. This includes for example health data and trade union membership.
When we process your personal data that requires your consent, you have the right to withdraw your consent at any time.
We will not disclose your personal data that is collected with this form to third parties.
We do not use automated data processing or profiling.
Your personal data is stored for 6 months or for as long as other statutory obligations require.
If we want to process your personal data in the future for a purpose other than that for which it was collected, we will inform you about it.
We will always endeavour to keep your personal data correct and up to date. Please note that you also bear responsibility for ensuring that your personal data is up to date.
Your rights
You have many rights concerning your personal data. You have the right to access your data, the right to rectification or erasure of your data, the right to data portability, and the right to object to or restrict the processing of your data.
